Centre injury/incident report form
For injuries where medical attention is sought or an ambulance is called, details should be entered on an insurance form.  This must be done at the time of injury and be available, even if there is later found to be insufficient grounds to submit a claim.
For injuries/incidents of a more minor nature, a record should be kept by the Club/Centre or LAWA (for LAWA Events) in an incident register.  A report form is available here.
